help diagnose any underlying medical or biochemical origins of
disease. This comprehensive assessment includes:
- psychiatric
evaluation
- history and physical
- nursing assessment
- psychosocial
history
- recreation therapy assessment
- patient/family
educational learning assessment
As indicated by patient need, the
physician may order additional assessments including:
- psychological
testing
- nutritional assessment
- laboratory
- radiology
- vocational
rehab
- speech/language and hearing assessment
An individual's
treatment plan will focus on attainable short- and long-term goals
established with the input of the patient, the patient's family and
referring professionals. Active family involvement
in treatment is a critical component as the patient's recovery is
often correlated to healthy family interaction.
The physician and social worker
coordinate the discharge plan with the patient, their family and the
referring professional.
