An individual’s treatment plan will focus on attainable short- and long-term goals established with the input of the patient, the patient’s family and referring professionals. Active family involvement in treatment is a critical component as the patient’s recovery is often correlated to healthy family interaction.

The physician and social worker coordinate the discharge plan with the patient, their family and the referring professional.
